How to access our NHS vasectomy service

If you are a Frimley ICB patient, you can refer yourself directly for a vasectomy using our secure online self-referral form below. It’s quick and convenient – just make sure to enter the name of your GP practice in the form. Referrals without the GP practice name cannot be processed. Before you start completing the form, please read this whole page completely.

Please do not include any clinical or medical details in the self-referral form, as it is not intended for sharing personal health information.

If you are registered with a GP under BOB ICB, you are not yet eligible to use the self-referral form. Instead, please contact your GP and ask them to refer you to our NHS vasectomy clinic. We are working to expand self-referral access to BOB ICB patients in the near future.
